For a nominal fee, you can choose from a menu of tests including: lipid profile, complete blood count (CBC), cholesterol, ABO & Rh blood type, fasting glucose, hemoglobin A1C, pregnancy test, strep screen, urinalysis, among others.
How Does it work?
You may pick up a form at the Port St. John or Titusville location or download this form, fill it out and return it to either of these locations. Choose from the list of tests you would like to have performed. If you would like a form faxed to you, please call 268-6134.
