- First Health - PPO (includes CCN, SouthCare)
- Healthease – Medicaid HMO (part of Wellcare)
- Health First – Commercial, Brevard Board of County Commissioners, HMO and Medicare HMO
- Health Options – HMO (Blue Cross Blue Shield Of Florida)
- Health South Corporation – PPO
- Humana Choice – Medicare PPO (a Humana Medicare Replacement product)
- Humana Gold Choice – Medicare PFFS (a Humana Medicare Replacement product)
- Medipass - Florida Medicaid HMO
- Multi Plan, Inc. – PPO and Worker’s Compensation (owns PHCS and Beechstreet)
- National Preferred Provider Network (NPPN) - PPO
- Preferred Health Network – Worker’s Compensation
- Private Health Care Systems (PHCS) – PPO (Purchased by MultiPlan 10/06)
- Rockport - Worker’s Compensation
- SouthCare – PPO (Purchased by First Health)
